As a courtesy, we will bill insurance directly if your policy includes Massage and/or Acupuncture benefits. Fill out the Benefit Check form below prior to your appointment so we can verify benefits! Please fill out a 2nd benefit check form if you have a secondary insurance.

Payment is dependent on eligibility of services at time of service and all terms and conditions of your plan. You are responsible for any charges not covered by insurance.
